Dependent Life Insurance
This benefit is available to you only if you
also elect Associate Supplemental Life Insurance. Dependent
Life Insurance can help you pay for funeral expenses
or provide additional income to you in the event of
the death of your spouse, domestic partner or child.
Although it's painful to think about, it is important
to consider the potential financial impact of a spouse's
or dependent's death. For example, if both you and your
spouse work, the death of your spouse could cause significant
finacial hardship. On the other hand, if your spouse
stays at home and cares for your children, you would
need to pay someone to take on those responsibilities
in the event of your spouse's death. Dependent life
insurance can help with those financial needs
Who Can Be Covered
You can purchase life insurance on your legally
married spouse or domestic partner and your unmarried
children. Your unmarried dependent children are eligible
for coverage from 14 days old up to the end of the year
in which they reach the age of 19. If they are full-time
students they may be covered to the end of the month
in which they reach the age of 25 or the end of the
month in which they are no longer enrolled as a full-time
student. Dependent Life Insurance can help you pay for
funeral expenses or provide additional income to you
in the event of the death of your spouse, domestic partner
or child. Coverage may be kept in force for any handicapped
child who reaches the age limit if they become unable
to earn a living before reaching age 19, or before age
25 if they are a full-time student. Application and
documentation of handicapped status must be provided
prior tot he cancellation date that normally occurs
for dependent children. Continuation of coverage is
not activated until approval is made by the individual

Coverage Options
You have the following coverage options:
- Coverage on your spouse or domestic partner
- $ 5,000
- $10,000
- Coverage on each dependent child
- $ 2,000
- $ 5,000
- $10,000
Evidence of Insurability (EOI) requirements apply if
you enroll after your initial eligibility period (30
days after you were hired or become benefits-eligible
for the first time) or choose to raise your coverage
level. If the level of coverage for which you apply
requires completion of an EOI, you are not covered for
that amount until you receive written notice of approval
from the insurance carrier.
Beneficiary
You are the beneficiary for benefits paid after the
death of a covered family member.
Cost of Coverage
The cost of coverage under the Dependent Life Insurance
Plan varies, depending on whether it is coverage on
your spouse / domestic partner or your dependent children.
The cost is deducted on an after-tax basis from every
bi-weekly paycheck during the covered period.
