<p>The American Cancer Society reports that cancer strikes three out of four families. The Health Alliance is making available to you a Cancer Expense Insurance Policy that not only provides payments if you are diagnosed with cancer, but also offers coverage for wellness screening. </p> <p><strong>Coverage Options </strong><br> There are two options available to you, your spouse or domestic partner, and your children to age 25 (if full-time students). For definition of domestic partner, see <a href="eligibility.htm">Your Eligibility for Health Alliance Benefits</a>. </p> <p>Benefits provided by this insurance include:</p> <ul> <li><strong>Series A-59300</strong> - pays first occurrence of internal cancer diagnosis and treatment benefit of $5,000.<br> “Internal Cancer” includes melanomas classified as Clark’s Level III or higher. </li> </ul> <p>Additional benefits under both plans include:</p> <ul> <li>Hospital confinement - $300 a day for the first 30 days, then $600 a day thereafter for inpatient. </li> <li>Radiation and chemotherapy – up to $300 a day for charges incurred for the modification or destruction of abnormal tissue.</li> <li>Experimental treatment – up to $300 a day for charges incurred for experimental cancer treatments for the modification or destruction of abnormal tissue.</li> <li>Anti-nausea benefit – up to $100 a month.</li> <li>Nursing services – up to $100 a day for full-time care by RNs, LPNs or LVNs.</li> <li>Surgical/Anesthesia benefit - $100 to $5,000 (depending on the type of surgery) when a cancer-related surgical operation is performed.</li> <li>Prosthesis benefit – up to $3,000 for charges incurred for surgically implanted prosthetic devices.</li> <li>Wellness benefit – up to $75 per calendar year/per person for charges incurred for any one of 12 cancer-screening tests, after 30 days of coverage is in force. </li> </ul> <p>Other benefits payable under a payment schedule include:</p> <ul> <li>Outpatient blood and plasma (up to $250) </li> <li>Transportation ($.50 per mile up to $1,500) </li> <li>Bone marrow transplant (up to $10,000) </li> <li>Extended care facility ($100 day/365 day max.) </li> <li>Home health care ($50 per visit/30 visits a yr.) </li> <li>National Cancer Institute evaluation and consultation benefit ($500) </li> <li>Second surgical opinion (up to $250) </li> <li>Lodging for out of area ($60 per day) </li> <li>Stem cell transplant (up to $2,500) </li> <li>Ambulance (charges incurred within 100 miles) </li> <li>Hospice ($100/day for 60 days, then $50/day) <br> </li> </ul> <p>For information on annual and lifetime limits, contact <strong>Chuck Hais at Brecek & Young Advisors, Inc.</strong> at <strong>513-721-2513.</strong> Full coverage (other than First Occurrence Benefit) is available even if you have had cancer in the past as long as you have been in remission at least five years. </p> <p><strong>Cost of Coverage </strong><br> The cost of coverage varies depending on the plan you choose and who is covered. Refer to the cost sheet for specific premium amounts. Premiums are deducted on a pre-tax basis from every paycheck.
